Health Buddy Program:

1.1 The senior participant grants permission for the Health Buddy to accompany them during medical appointments.

​

1.2 During appointments, the Health Buddy will take notes on behalf of the participant for their reference.

 

1.3 The senior participant understands that the Health Buddy's role is limited to providing assistance during appointments and does not include decision-making, medical advice, or medication pickup.

 

Non-Disclosure:

​

2.1 The senior participant agrees that the Health Buddy will not disclose medically related information shared during appointments to any third parties without explicit consent.

 

2.2 The senior participant trusts the Health Buddy to maintain confidentiality and avoid unauthorized disclosure.

Safety and Reporting:

​

3.1 In case the Health Buddy encounters a situation during an appointment that raises concerns for the participant's safety, they may report it to Health Buddy executives or appropriate authorities.

 

3.2 Health Buddy is not liable for consequences resulting from reporting such situations, as long as it is done in good faith and prioritizes participant safety.

 

Indemnification:

 

4.1 The senior participant agrees to hold Health Buddy harmless from any claims or liabilities arising from medical appointments or services, except in cases of gross negligence or intentional misconduct.

 

Limitations:

5.1 The senior participant understands that the Health Buddy cannot make decisions or provide medical advice.

​

5.2 The senior participant agrees to consult their healthcare provider for medical advice or decisions.

 

By completing a request for a Health Buddy, I acknowledge that I have read, understood, and agreed to the terms and conditions of this Confidentiality and Medical Appointment Assistance Agreement as a senior participant.
